Hospitals and medicine

Seeing a doctor at a hospital, what treatment costs, prescriptions and repeat prescriptions, the Paleto Bay Pharmacy, the conditions and allergies you choose in character setup, the Health app, plastic surgery and what happens to a body. For anyone with a character.

What it is

Every hospital in the city has a desk where you can check in for treatment or see the duty doctor. The doctor asks what brings you in, examines you, treats what is really wrong and can write a prescription for antibiotics, pain relief or a regular medicine. Prescriptions are real records with your name on them: you collect them in person with your ID card, controlled medicines are logged in a register, and long-term conditions get a repeat prescription every week without you asking.

Your character can also live with a condition from day one. Character setup has an optional health step where you pick conditions and allergies, and from then on the city treats you accordingly: your repeat medicine is issued automatically, pharmacists will not sell you anything you are allergic to, and doctors see your record when they treat you.

How to start

Walk into any hospital and press E at the desk ("to see the hospital desk"). The hospital panel opens with your triage badge, your active injuries, a Medicines tab and a "See a doctor" button. Every character starts with an ID card in their inventory; keep it, because you cannot collect a prescription without it.

A consultation with the duty doctor is charged by what you came in for. The fee arrives as a medical invoice, which you can pay from the Invoices tab of the hospital panel. If the doctor finds nothing wrong with what you described, you pay the smaller "nothing found" fee.

Step by step

  1. Walk in and see a doctor

Press E at the hospital desk and choose "See a doctor". One of the duty doctors greets you and reads your vital signs out loud.

  2. Say what brings you in

Pick an answer: you have been hurt, you are not feeling well, you keep getting headaches, it is about something ongoing, or just a check-up. The doctor asks a follow-up question, such as which part of your body hurts.

  3. Get examined

The doctor examines what is really there. Only real injuries are treated, and only the part of the body you named. Claim an illness you do not have and you pay the "nothing found" fee.

  4. Treatment and the invoice

Wounds are cleaned and treated on the spot. The summary tells you what was found and what was done, and the fee is invoiced to you. A check-up that finds nothing wrong also tells you your blood type.

  5. Any prescription

If you need medicine, the doctor writes a prescription: antibiotics for an infected wound, codeine for a moderate injury (unless you are allergic), sumatriptan for a diagnosed migraine. The prescription goes on your record straight away.

  6. Wait for the notification

When a prescription is ready you get a notification on your phone and on screen naming the medicine and where to collect it.

  7. Collect it with your ID

On the English server go to the Paleto Bay Pharmacy and press E "to speak to the pharmacist", then open the Prescriptions tab. On the Balkan server open the hospital desk and use "Open the counter" on the Medicines tab. Only the named patient can collect, and the ID card must be yours.

  8. Repeats look after themselves

If the prescription is for a long-term condition, the next supply is issued automatically a week later and the notification comes again.

Prescriptions and repeats

A prescription is either acute (a one-off course, such as antibiotics) or a repeat for a long-term condition.

Review

None

A medication review is due every 56 days

  • Supplies never pile up. If you miss a week, the next supply replaces it rather than stacking.

  • Missed doses only bring mild roleplay reminders, such as feeling anxious, low, shaky or short of breath. They never hurt you or knock you down.

  • The medication review is due every 56 days, with a reminder each week once it is due. Your repeats carry on meanwhile. See a doctor and choose "Review my existing condition and medication" to renew it.

  • Once-a-day medicines such as antidepressants, insulin and blood pressure tablets count one dose a day, so a double click never wastes tomorrow's dose.

Controlled medicines

Some medicines are controlled: methylphenidate (Ritalin), mixed amfetamine salts (Adderall), alprazolam (Xanax), clonazepam (Klonopin), diazepam, zolpidem (Ambien), gabapentin, tramadol, codeine and oral morphine. They carry extra checks:

  • Your ID is checked every time you collect.

  • Collections of the same controlled medicine are at least 6 days apart.

  • Every collection is written into a controlled drugs register.

For doctors. Player doctors prescribe from the Emergency Services Portal with /esp: a searchable formulary, conditions and repeat prescriptions, allergy clash warnings, cancelling with a reason, and a controlled drugs register tab. Only senior doctors can prescribe controlled medicines. See Factions and the MDC for how the emergency services work.

The Paleto Bay Pharmacy

The pharmacy is on the English server only, next to Paleto Bay Medical Center. Press E at the counter "to speak to the pharmacist". The panel is signed by the duty pharmacist, Maya Castell, although nobody stands behind the counter. It has three tabs: Shop, Prescriptions and Advice.

The Shop tab stocks 33 over the counter lines from $3 to $35: pain relief (paracetamol, ibuprofen, aspirin, naproxen, ibuprofen gel, co-codamol), melatonin and a night-time sleep aid, cold and flu sachets, decongestants, nasal spray, cough mixtures, lozenges, hay fever tablets, stomach remedies, plasters, bandages, first aid pouches and creams, eye drops, nicotine patches and gum, vitamins, a pregnancy test and a thermometer.

  • Medicines carry no sales tax. Pay by cash, card or bank.

  • A basket holds up to 12 lines, and each line has its own limit per sale.

  • Some lines have an age limit of 12, 16 or 18.

  • Anything you are allergic to is marked "Not suitable for you" and the pharmacist will not sell it.

Three pharmacist medicines are limited to one pack per person every 24 hours:

On the English server, 24/7 stores no longer sell antibiotics. They are prescription only: see a doctor, then collect them here.

Conditions and allergies from character setup

When a new character first arrives in the city, character setup walks through their background, personality, health, finances, citizenship and chat. The health step is optional: pick up to 5 conditions, each with the date the character was diagnosed in their story (typed as DD/MM/YYYY), and up to 4 allergies, each with a severity of mild, moderate or severe. If your character has none, press "Skip: no conditions or allergies"; that button only shows while nothing is picked.

Each condition shows what it means in play and its regular medicine. A controlled medicine is marked Controlled.

Allergies

No regular prescription: buy allergy relief at the pharmacy

The allergies you can record are penicillin, anti-inflammatory painkillers (NSAIDs), opioid painkillers, latex, peanuts, shellfish, bee and wasp stings, and pollen.

  • Conditions you pick here are on your medical record from day one, and their repeat prescriptions start automatically with the first supply free.

  • If your character's conditions were recorded before this system existed, their repeats start too, but a controlled medicine is swapped for an uncontrolled alternative where there is one, or left for a doctor to start.

  • A severe allergy is roleplay only. Nothing in the city triggers a reaction automatically.

  • The duty doctor can also diagnose some conditions later: depression, generalised anxiety, PTSD, panic disorder, OCD, social anxiety, insomnia, migraine and hay fever. A new diagnosis books a free follow-up appointment two weeks later.

Your hospital visit

  • Triage. The hospital panel shows a triage badge from Immediate to Non-urgent, based on your injuries.

  • Medicines tab. Your prescriptions, what is ready to collect and where, with a button to set your GPS to the pharmacy on the English server.

  • Discharge summaries. After treatment you get a summary of what was done. After a severe injury it includes a fit note of 2 days off work and no strenuous activity; after a critical one, 3 days.

  • Free follow-up. Serious treatment or surgery books a free follow-up appointment two days later, and it appears in your phone calendar.

  • Ambulance handovers. When paramedics hand you over at a hospital you are admitted with an invoice, not left at the door.

  • Refunds. The hospital only charges for treatments it offered you, and switching character or leaving mid-treatment refunds you.

On the English server, hospital charges, invoices, consultations, booking fees and pharmacy sales go to the medical faction's bank.

The Health app

The Health app on your phone shows your prescriptions with their status and where to collect them, your repeat prescriptions, your allergies and your recent visits. See The phone for the rest of the apps.

Plastic surgery

Plastic surgery changes your character's face and appearance in the character creator. Stand at a clinic and press E "to change appearance". It is refused while you are in a vehicle, injured or jailed.

Bodies

A dead character leaves a body that is an exact copy of them: the same face, clothes and pose where they fell. Walk up to it and press E "to examine the body" to see the cause of the injuries and roughly how long ago it happened.

A body can be searched with /lootbody from within 3 m, but only once it has been there for five minutes; until then you are told the body is still warm. Each search takes one stack of items and everyone nearby sees you rummage through the corpse. Emergency services and the coroner can collect a body; otherwise it is removed after two hours.

RP SCENE

Sophie Marsh has asthma, picked in character setup when she arrived on the English server a week ago. Her first inhaler was free. This morning her phone buzzes: "Your prescription for Salbutamol 100 mcg inhaler is ready to collect at Paleto Bay Pharmacy, next to the Medical Center. Bring your ID card."

She walks down from Paleto Boulevard and presses E at the counter. The pharmacy panel opens on the Shop tab; she switches to Prescriptions. Her inhaler is there with "Flat prescription charge" beside it: $10, her second supply.

/me slides her ID card across the counter and waits.

/do The card shows Sophie Marsh, and the face matches.

She pays $10 from her bank and the inhaler goes into her inventory. Back on the Shop tab she adds a pack of hay fever relief (loratadine 10 mg) for $5, because the pollen has been bad all week, and pays again. The confirmation reminds her that medicines carry no sales tax.

/me pockets the inhaler and steps back out into the sunshine.

What you see: the inhaler and the hay fever tablets in your inventory, $15 less in the bank, and the Health app on your phone showing the repeat as collected and the next supply due in seven days.
